International Health Certificate Submission Form

Submit all the necessary documentation for your international health certificate for your exam with one of our USDA accredited veterinarians. Please fill out one form for each patient that needs to travel. USDA certificates can cover multiple pets, but for our hospital's information each pet should have its own submission for review.

  • Microchips can be scanned any time we are open, no appointment necessary.

    • We recommend you bring your pet to our hospital to have their microchip scanned at LEAST 22 days prior to your travel date.
    • Proof of date of microchip implantation is often required. Please upload any invoice or medical record you have showing a date and your pet's name and microchip number. Failure to do so may require a new rabies vaccine and a delay of your ability to travel.
    • Microchips can stop working. To travel, your pet may require a new microchip and rabies vaccine and a delay of your travel date by 3 weeks.
    • Missing microchips may result in forced quarantine of your pet for 3 weeks in your destination country.

  • You may need to contact a previous veterinarian for older rabies certificates that include the necessary information
    International travel requires that your pet:
    • Have a valid microchip
    • Received a rabies vaccine while already microchipped
    • Is up to date on that rabies vaccine or
    • Did not have a rabies vaccine lapse since then
    • That the microchip is present and active (will be scanned again at your appointment)


    Rabies Certificates MUST include a microchip number, name and signature of veterinarian, as well as the Rabies Vaccine's:

    • Name
    • Manufacturer
    • Batch Number
    • Administration Date
    • Duration of vaccine validity (1-3 years)
    • Type of vaccine (killed, modified live, etc.)


    Please upload your pet's most recent, unexpired rabies certificate as well as the rabies certificate most recently before it. If there has been a lapse, your pet's most recent rabies certificate is considered a new "primary" rabies vaccine.

    Please fill out this Health Certificate Information Form to let us know the relevant details regarding your travel plans. This will allow us to begin preparing your official certificate in advance, cutting down on your appointment time and ensuring we have all information needed.

  • USDA Requirements by Country

    Schedule your international health certificate at least appointment 4-5 days prior to your travel date due to the time it takes for the USDA to review and/or mail your certificate.
    Failing to do so may result in your certificate not arriving in time for your flight.
    Please check with your airline and check your destination country's time requirements at the link above to ensure that your appointment is scheduled appropriately.
